About this service

The Northern Melbourne Psychosocial Support (NMPS) service offers individual and group support for people who live with ongoing mental health challenges that affect their daily life, and do not access the National Disability Insurance Scheme (NDIS). We work with people to strengthen their connections to the community and support networks. People can meet with us at our service sites or in the community.

NMPS offers two tailored services to strengthen wellbeing and access to care.

  • Integrated Chronic Care (ICC) helps people understand and manage physical health needs, reduce risks of chronic conditions and improve health conversations through collaborative planning with a nurse and peer worker. You can opt to complete the Health Prompt with ICC staff members and your support worker or not at all. We collaborate as a team with people to address barriers and identify resources for appropriate and sustainable health care.
  • The NDIS Interface service supports people to explore or re-test eligibility for the NDIS, assisting with assessments, evidence gathering and applications, while linking to other supports if it feels right for the individual.

Both services work alongside NMPS support staff to ensure care is coordinated and responsive to individual priorities.

The Northem Melbourne Psychosocial Support Service offers community-based, one-to-one support and group activities that assist people experiencing wellbeing and mental health challenges.  

Support to suit your needs

We provide support based on your needs and preferences. This can include developing strategies and skills to: 

  • reduce distress and manage symptoms 
  • connect with (chosen) family, friends and community
  • improve confidence and build independence 
  • enjoy a full and vibrant quality of life. 

Group support

We facilitate group programs, incorporating wellbeing and mental health expertise on a wide range of topics.

Group programs focus on supporting people in the following areas:

  • learning new skills and building on existing strengths
  • developing confidence with community living
  • increasing structure in daily routines
  • improving emotional wellbeing and physical health
  • identifying activities that reduce loneliness and support social connection.

Who is this for?

This service is free and available for people: 

  •  who experience mental health challenges that impact their quality of life (no diagnosis required) 
  • who are not funded through the National Disability Insurance Scheme (NDIS)
  • living in or connected to the following council areas:
    • Darebin
    • Hume
    • Moonee Valley
    • parts of Macedon Ranges
    • Melbourne
    • Merri-bek
    • Yarra.

What to expect

Accessing the service

Anyone can make a referral, including: 

  • you (our intake workers can support you to complete a referral form)
  • a (chosen) family member, carer or friend
  • your doctor or General Practitioner (GP)
  • mental health workers or other health care providers. 

Please discuss referrals and consent with any person you’re referring to the service before contacting us. 

We encourage referrals from people from multicultural and multifaith backgrounds, Aboriginal and Torres Strait Islander peoples, people who identify as sexually and gender diverse, refugees and young people. 

Referrals into this service are likely to join a waitlist. We encourage anyone wishing to enquire about the program to contact us directly.
