What is the Health Prompt?
The Health Prompt is a practical tool used to facilitate meaningful conversations about physical health with people accessing mental health and social services.
It was created to meet the needs of people experiencing mental health challenges, who often experience higher rates of physical illness, delayed diagnosis and reduced access to care.
How was it developed?
How do I use it?
The Health Prompt is designed to be completed collaboratively by staff and the individual they’re supporting.
It includes a structured set of questions, but it is not a formal assessment. Instead, it’s a conversation tool to support respectful conversation, reflection, choice and shared understanding.
We recommend using the Health Prompt as part of ongoing conversations about wellbeing – ideally every six months, or more often if health needs or circumstances change.
