Service coordination

Finding the support services that work well together and provide you with the right support, are vital to effective recovery.

Kamsiah and Philippa

Through support coordination, we can assist you to navigate the different services available and help coordinate a range of supports that are right for you, including:

  • Public, community and private mental health support
  • Psychiatrists and psychologists
  • Primary health including GPs and community health services
  • Alcohol and other drug treatment, disability, housing, family violence and other community services
  • Centrelink and income support
  • Training, education and employment
  • Parenting support and child protection

We work with people to identify how services can be better coordinated and strengthen the way they work together. By assisting services to work in a more collaborative, coordinated and integrated way for people with complex needs, we can help to ensure you receive high-quality, person-centred, equitable and accessible support.
