Neami delivers new homelessness service in Frankston and Geelong

We’re pleased to announce that Neami is operating a new homelessness service in Frankston and Geelong called Towards Home+. The service provides support for people who are at risk of homelessness or experiencing homelessness and is now accepting referrals.
Towards Home+ is being delivered by a multidisciplinary team comprising support workers, peer support workers, youth workers, community health nurses, Aboriginal and Torres Strait Islander Health Practitioners, and AOD workers.
The service uses a shared-care approach to support people to obtain and maintain housing, improve physical and mental health, and develop connections in the local community.
The Assertive Outreach Team engages people who are sleeping rough and focuses on brief interventions and short-term support, including rapid access to emergency accommodation and health services as well as access to stable accommodation (HousingFirst approach).
The Supportive Housing Team supports people to remain housed and provides longer-term intensive, flexible and consumer-centred support including development of daily living skills, participation in social activities, case management, and care coordination to facilitate multi-agency ‘wrap-around’ support.
Six modular units will also be available in Norlane, Geelong as part of the service, and will provide a recovery-focused environment that will encourage residents to engage with local community and service networks. The Modular Units Team will provide flexible individualised support to ensure the secure transition of all residents to sustainable long-term housing within a two-year timeframe.
In Frankston a Supportive Housing service will be offered. In Geelong the service program will include Supportive Housing, Assertive Outreach and modular housing.
Towards Home+ is supported by the Victorian Government and delivered by Neami National in partnership with Launch Housing, Wathaurong Aboriginal Co-op, Uniting Care ReGen, Bolton Clarke, and Melbourne City Mission.
