Together Home provides access to safe, secure and affordable housing and the support needed to help sustain a tenancy.
Access to housing and support through the program works to address people’s needs while building individual capability and capacity. By understanding each person’s needs and goals, Together Home also fosters local connections to support people to thrive in the community.
Together Home is a key initiative supporting the Premier’s Priority to halve street homelessness by 2025. This initiative aims to transition people onto a trajectory away from homelessness and into long-term, stable housing, whilst improving overall personal wellbeing.
National Psychosocial Support Measure - Extended
We offer individual coaching and group activities for people who are transitioning from the Partners in Recovery service and who are not eligible or have not applied for the NDIS.

- Are not eligible or have not applied for the NDIS
