About Health Service Navigator

Understanding and navigating the pathways to mental health support can be complex, particularly when people are feeling unwell. 

The Health Service Navigator acts as a single point of entry to assist people in finding and engaging with mental health support. 

The Health Service Navigator assists GPs, service providers, consumers and carers to navigate the services available under Stepped Care in the Ipswich, Toowoomba, Scenic Rim and Lockyer Valley regions. 

Who is this service for?  

Anyone seeking assistance to manage their mental health concerns can access the service. 

Mental Health Service Navigators facilitate the intake process, referrals and enquiries before connecting people with the most appropriate local service for support. 

Neami provides Mental Health Service Navigators to people across the Ipswich, Toowoomba, Scenic Rim and Lockyer Valley regions. 

 

Benefits 

The Health Service Navigator’s role is to:  

  • Coordinate linkages to supports and services for patients and consumers;  
  • Map and identify mental health service needs and gaps in communities at the local level;  
  • Provide information where appropriate for services offered by the Darling Downs-West Moreton PHN under Primary Mental Health Care and information for consumers on other supports as necessary. 

What to expect

As part of the Stepped Care approach to mental health, the Health Service Navigator links people with local services and supports them to engage with the services that most suit their needs. 

The service is staffed by Mental Health Service Navigators, who bring a wealth of experience assisting people to identify and explore their mental health needs. Comprehensively trained in recovery orientated practice and service navigation, the team complement the support offered by available Stepped Care services. 

In addition to specialised mental health support experience, Mental Health Service Navigators also have a comprehensive understanding of the available local support services and their unique programs.

Referrals by GPs and Primary Health providers can be made can be made using the forms available on the Darling Downs and West Moreton PHN website and faxing the form to 07 3103 4634.
