The Neami MaCNI team provide:

  • Care Plan Assessment and Development
  • Care Plan Coordination

Multiple and Complex Needs Initiative (MaCNI), provides targeted, time-limited and flexible interventions for people aged over 16 years, who have complex needs such as a combination of mental illness, substance dependency, intellectual impairment, acquired brain injury, and who pose a risk to themselves and/or others.

MaCNI aims to increase positive connections with support systems in local communities including primary healthcare, housing, education, alcohol and drug services and the criminal justice system. An individually tailored wrap-around care plan assessment and care plan development that aid recovery are key to the MaCNI program.

The Neami MaCNI team offer Care Plan Assessment and Development, as well as Care Plan Coordination.

A MaCNI Care Plan is developed based on a comprehensive assessment, including meeting with consumers, their family and support networks. The Neami MaCNI team are highly skilled and experienced in Care Plan Assessment and Development and adopt Neami National’s recovery-based and evidence-informed approach to person-centred practice.

The Neami MaCNI team are experienced in working collaboratively across agency and as part of multidisciplinary teams to support consumers with complex needs, this is key to providing state-wide MaCNI Care Plan Coordination.
